of adult ADD/ADHD/EF disorders, anxiety, and depression. We provide support to adults (18+), couples and parents

Confidentiality Information for Counselling Provided by Bmindful Therapists in Ottawa

Purpose of Collection of Personal Information

Your personal information is collected for the purpose of assessing your needs, conducting an evaluation of your presenting concerns and for providing personal and/or group psychotherapy. Hard copies of your information are stored in a locked cabinet and kept electronically under password protection.

Client Records

The psychotherapist will securely retain client information for 10 years beyond your last counselling appointment. All information will be disposed and shredded after this time period. The client is permitted access to the hard copy of their file by completing and signing a request to obtain file information. However, for record purposes, the client is not permitted to remove or destroy contents from the file.

Disclosure of Personal Information

Your psychotherapist can disclose personal information with formal written consent. With your consent, your personal information may be shared to a third party. Personal information may also be shared with staff and supervisors for the purpose of supervision or professional consultation.

Risks of Electronic Communication

The therapist and the client will take necessary measures for electronic practices (e.g. security software, encryption and password protection) to ensure the confidentiality of electronic communication. These practices are meant to reduce the possibility that transmission of information may be accessed by another person. Despite these practices, there are risks to communicating via online services (e.g. email, videoconference). Given the nature of electronic communication, all information and exchanges may not entirely be secure.

Limits to Confidentiality

What you share with your psychotherapist is private information and will be kept confidential. However, there are some exceptions to the therapist’s duty to maintain confidentiality listed below:

If the psychotherapist learns that a child is or may be at risk of abuse, neglect, or in need of protection,
If there is a case of sexual abuse by a regulated health professional,
If the psychotherapist believes you or another person is at risk of imminent harm, and
For the purpose of complying with a legal order such as a subpoena, or if the disclosure is authorized by law.

Accountability

Psychotherapists are members of the College of Registered Psychotherapists of Ontario. They may also be members of the Ontario Association of Consultants, Counsellors, Psychometrists and Psychometrists (OACCPP) or the Canadian Counselling and Psychotherapy Association (CCPA).
